What We Do...
Case Management Services

Families needing services available through the Circle of Care Center for Families are invited to call or come by the office to talk with the Center's Case Manager. Beginning with an individualized family assessment, the Case Manager partners with a family to determine their needs, their strengths, and their goals. The Case Manager, then, works intensively assisting the family with locating their needed services. Some families need an emergency or short-term type of assistance, while others may need on-going services and family support that will last for several weeks or months.
